Important Disclosure
Choice Dental Membership Program is available to insured and uninsured patients of our practice and are not pre-payment for future services or payment for access to discounted services. Instead, our patients elect to make annual subscription payment for discounted dental care versus paying at the time of each service.
The membership plan is a direct agreement between the patient and the legal entity of our practice. Membership plan payments must be current in order for the patient to receive the benefits included with their membership plan.
Membership plan benefits may not be combined with any other offers or discounts. Payment for additional treatment provided by our practice to the patient is due at the time of service and our membership plans do not cover procedures completed outside of our practice.
The term of our membership plans is 12 months from the patient Effective Date and automatically renews on the Renewal Date. Patients can opt-out of membership plan renewals with 30 days notice by clicking “Cancel Plan” in the Member Portal or by contacting our practice directly. No refunds will be provided for membership plan subscription fees paid under any circumstances.
Please note that the following are not included in these plans:

- Implants
- Laser Surgery
- Full Mouth Reconstruction
- Crowns and Bridges
- Orthodontics (Braces)
- Dentures
- Whitening (Bleaching)
- All-on-Four or All-on-Six
Family plans under any category covers a family of 2 children up to 16 years old. If there are more children, 30% of the plan would be paid on each child.
Non-members cannot be swapped in place of members of the family initially registered. Only members will be attended to.
We reserve the right to make changes to our Dental Membership Plans when we consider it necessary to do so.
